I'm applying to medical school this cycle and I just noticed the institutional action question. It says:
"Were you ever the recipient of any institutional action by any college or medical school for unacceptable academic performance or conduct violation, even though such action may not have interrupted your enrollment or required you to withdraw? You must answer "Yes" even if the action does not appear on or has been deleted or expunged from your official transcripts due to institutional policy or personal petition."
During my freshman year in the dorms I was written up for violating the school's guest policy (I guess I had too many people in my room) and underage alcohol policy. I had to meet with an officer at the housing office and write an essay and was on "probation" for a few months, although I didn't notice it's effects whatsoever. I asked the housing office, and they said they still have a record of the incident. I also called the registrar, and it is not on my official transcript. I'm trying to get in touch with the Dean of Students to decided whether or not to put it on my application. Obviously, I'd rather not.
What do you think? If I do end up putting it on my application, will it hurt my chances?
Absolutely agree with my esteemed colleague. This is harmless, but you can't afford NOT to omit it. Just explain it and own it and you'll be fine.
