If you are accepted by a phone call - can you officially accept an offer without a written document? Do you have to wait until there is an official offer to accept? I'm a little confused... I'm excited but there's no paper evidence!

I believe (but could be mistaken) that a phone offer should theoretically count as official. However, I wouldn't rescind my submitted application at other programs until I had an emailed copy of the offer at the least.
 
although i was offered a spot on the phone which i accepted verbally, my poi told me that i had to accept their offer in written form (email was okay) for it to be official. perhaps that was just my program though 🙂

It is always better to have something in writing. So, I agree. On the other hand, when we got phone calls for interviews...did we wait for a letter?

Accepting the offer over the phone should be fine. However, as PsychAndMusic mentioned, many programs will require formal written acceptance for their records.

Also, interviews are a much different situation--by accepting an interview offer, you aren't automatically disqualifying yourself from accepting other interview offers, nor are there other contractual components involved (e.g., stipend and tuition remission, eligibility standards and maintenance, etc.).

...right; I was more thinking of us trusting that an offer made over the phone is as valid/true/happening as a letter and compared that to the invite for an interview made over the phone where we do trust that it is 100% true that we indeed are invited. So, just a different level of thinking about it.
