Short answer, probably.
Avg score goes up each year. There are many candidates with good rank AND high CBSE score.

So you have to have something else that makes you stand out.
For example, good LOR and being likable during interviews will do.

In the most recent JOMS, there’s a table that shows the average CBSE score over time. It has been rising year after year. Not to mention more and more people are applying. I’m not even sure a 70 is all that competitive anymore. The way things are going, I think most people will need to do a preliminary year before matching.
 
I’ve done it, but it required a perfect storm of factors.

First, you need a good excuse and by that I mean pretty much anything short of a medical reason for a poor class rank isn’t going to cut it come interview season (and I still got a pointed question here or there). Solid LORs from academically active surgeons can actually help quite a bit here.

Second, an 80+ is pretty much non-negotiable. Apply broadly (like 30+ programs) and you might get some interviews, but 4 year programs are likely out of the equation.
 
it depends how poor of a rank/GPA. I did well on the CBSE but my rank was not where I thought it should be. I did get 4 and 6 year interviews and did not do a non-cat. You can message me if you have any questions!

Purely anecdotal, but many of us interviewing last cycle felt that CBSE scores were inflated due to COVID since students had a lot more free time. We don't really know the 2022 and onward landscape, plus the scoring system changed again so it's hard to compare. That being said I do agree that it's becoming more competitive and non-cats are almost becoming a pseudo-requirement (esp for 4 yrs).

Looking through the match data it does look like applicants are applying/ranking significantly more programs than they did a few years ago and programs vice versa.
 
All these zoom interviews also, enable interview hoarding, with a top applicants will go on tons and tons of interviews because they don’t have to travel, and that leaves less interview spots for everyone else.
