I think you've correctly identified your weaknesses. The shadowing issue is easy to fix. But I view top twenty med schools as generally being ranked as such due to their heavy emphasis on substantive research, and if you plan to apply in a year, you'll have less than two years of research to list. Further, even if you start in a new lab now, you'll maybe just be getting up to speed by a year from now where a PI would be trusting you to take on your own project.

Some do get into top schools due to amazing leadership and wow-level involvement in nonmedical community service. So the stronger you can be in those areas, obviously, the better.

That said, for general application purposes, I think you're doing a fine job of developing a solid application.

National Merit Scholarship (does this go on AMCAS?)
Since this is based on HS achievement, I'd say no.
 
Thanks Catalystik! Part of the reason why I posted this was to see if you noticed my W in orgo. Its a good sign that you didn't because I feel ADCOMs do the same thing in just glancing at everything. Do you think that significantly mars my application, or will ADCOMs just be "Ok, but meh...."? Thanks.
