I had a tentative shadowing position with a dentist but it wasn't definite until now. I e-submitted my application a couple months ago because I haven't heard from him and thought it would fall through. I called AADSAS and they said to mail it to the individual schools since we can't make changes after e-submitting. Does anyone know where to get the right mailing address and what to include to send them this attachment?

This is a fairly minor thing that I wouldn't really worry about. Maybe you can just include this information on any secondary applications you receive from the schools you applied to. If you really want to send this seperately to individual schools, here is a list of dental schools with addresses and contact info.
