I love the idea of being a doctor. I think its something I'll want to do with my life. But what if I become a doctor, start practicing and decide it's not for me? Am I just screwed?

What kinds of opportunities are there in this world for MDs that do not including actually practicing medicine?

There really are a world of possibilities for physicians and other health care professionals who wish to leave the clinical world and still make an impact in health care.
 
I would suggest figuring out if you want to practice medicine, or at least trying to, prior to starting medical school. Shadow some docs, talk to them about what the career is like. While there are non-clinical opportunities, med school is a huge investment of time and money. Better not to make the investment if you don't really want to use the license.

I would try to be as sure as possible I want to practice medicine before dedicating your life to it. It's not easy to leave medicine, since that's the one and only thing you are trained for. Not only are you losing potential income as well as accrue a gigantic debt, but you will also lose time in which you would otherwise be developing skills and experience to succeed outside of clinical medicine.
