Organization name on AMCAS for shadowing office-based PC doctor?

Hi everyone, I promise this will be my last AMCAS related post on SDN 😉!

I spent most of my shadowing experiences with an office-based primary care doctor. When listing his "0rganization name" under AMCAS, can I leave it blank or put the address.

I ask this because the office would not have an organization name as a hospital would, and just lists the names of the physicians outside.
