Same as D-school yo. they look at it all. class rank, board scores (for now), personal statement, letters of rec, etc. If everything else is okay except for that year off and you can justify why you had to take the year off you should still have a fighting chance. even if you got held back because of grades, if you kicked butt all other years and on the boards, go for it, what harm will there be? A friend got held back a year and is applying to OS this year, who knows what will happen!? My only advise would be to explain why in your personal statement, if you wait to explain at an interview I doubt you would even get there. Keep that statement positive, spin that years failure into something heroic! 😉
Depends on how badly that affected your class rank. I could see someone who was held back a year still getting in, if their class rank somehow climbed to the 5-10%. But unless you're in a class with slackers, I can't see how you would climb out of that hole after failing a year.

I think it depends on how obvious it appears on the transcripts. It could go unnoticed if the transcript is not looked at closely. If your numbers are good then I think you have a chance.
