OSU secondary error

whats the other two forms? The D.O. evaluation form and the preprofessional? Are we suppose to have our premed committees and the D.O. fill that out as well as our letters??
 
The DO eval form is not required as far as I know. The physicians can send it in with a LOR if they wish. Right now I can't get the webpage to pull up...I can't remember what the preprofessional form is.
 
The evaluation forms are optional, but my advisor recommended that I get them filled out --i guess they're used like a quick reference for the adcoms. I went ahead and had my evaluators fill them out and send them with their letters.
 
i dont think my premed committee would do it over again, and one of my interviewees no longer works there, so its gonna be a pain in the butt to get. so its not absolutely necessary?

I wouldn't sweat it, then. If you click on the link posted by goinverted, there's a word document you can open named "Supplemental application letter", and it says: "All letters of evaluation must be forwarded to the Office of Student Affairs by the individual writing them. The D.O. and Pre-Professional forms are optional and may be used for your convenience. "
 
Mailed my secondary a couple of weeks ago. Now I'm just waiting for my pre-med commitee and one of my recommenders (an OSU grad) to send their LORs. I really hope I get an interview 😳

I made it in and did not use the LOR form provided. I sent my LOR's using Interfolio.com.

It still amazes me how long this application cycle is for medicine. We haven't even started yet and the next crew is lining up.
