I got a msg in the mail from OSU telling me that i didnt send them their check; but i DID, 1 1/2 months ago. they lost it cuz it wasnt accompanied with the Mail-in-app-form on the web. for some reason i couldnot open that form so i called OSU; they told me its fine to send my check w/o the form. Now i am sending another check but w/o the form cuz that thing wont open. you think i should send my own form? or call them tomorrow ;its too late now.


if some one can access that form, can u please tell me if it requires anything other than SS#, ID#, DOB and signature?????


would appreciate it!

Yo yo,
I got the same mail notification. I checked my records and they had cashed my check over a month ago!!!
So I sent them an email..no one ever responding. I called them this morning and said they were having problems figuring it all out but that my file WAS COMPLETE and not to worry about that mail. Check you statement..make sure your check was cashed 🙂

At first it said i was missing 30 dollar check....
but then i checked it today and it was ok...
just call them and find out

Same story. I have a canceled check from 8/13, but they said they didn't receive it. I faxed them a copy of the check, and I am still waiting for them to change my status on their web page.
