Hello all,

I was offered an outpatient job at a CMHC, 5 x 8 hour days or 4 x 10 hour days..Adult patients, followups at 15 mins each and new patients at 1 hour each. Admin time was a measly 1 hour a week, however I am told that the clinic has a 30% no show rate, which I believe..

Is it standard to have some protected time daily for admin tasks/notes? If these are med-check appointments would it be reasonable to ask for 20 min slots, I realize I can ask for anything but I wanted to get some feedback from the forum..
 
How much are they paying? What’s the location? You left out important details
 
When people say 15 minutes for med management, is that the block of time in your schedule or with the patient? We have 30 minute (block, 20-25 with the pt) in residency and I can't imagine going shorter except with my least-chatty, stably-treated patients, who should mostly be discharged back to PCP at that point.
 
15 minute med checks is always a no go for me unless I'm the one running the practice. Add in very little protected admin time, and it sets you up to be abused. One hour per week is a joke, you will need at least an hour per day for paperwork. That supposed 30 percent no show rate will drop to 12 percent when the patients learn there is a doctor who cares working there now. At least, that's what happens to me. Also, every job has extra responsibilities or demands they won't tell you about until you've already moved into the position. Count on it.

Also CMHCs are usually desperate enough for psychiatrists that making 30 minutes for med checks a requirement in your contract is usually a doable thing .Have certainly met plenty of folks in agency work who successfully insisted on this as a condition of taking their jobs.

I’ve seen the opposite. They have a 30% no-show rate, so on average, you still get 20min per patient. That’s average for med management.

CMHC jobs are mostly about moving meat, not high quality care. Admin time is typically not given as you can type notes during appointments and answer questions/refills when no-shows occur. Admin time is something you generally don’t see much of in private practice or CMHC’s.

CMHC’s are generally under-funded.

Thanks for the response, not bothering posting reimbursement as i just wanted to see what the standard was for outpatient blocks/average followup times. That being said, what would you guys expect for such a set up in a rural part of the US, no on call/pager call required..

I think this must be very location dependent, one of the CMHCs around here even gives their therapists paid admin time.

I hope they are paying you more than 300k
In toledo cmh they were offering 300k: 20 minutes follow up with no show rate of 30%.
