The answer depends a little bit on which medical school you attend, and it depends a lot on how the rest of your grades look, your grade in your 4th year sub-i, and the strength of your letters of rec. Nothing is out of the question, but you should try to get an honors in your medicine sub-i and good clinical evaluations which will lead to a better dean's letter and letters of rec.

You can do a search on this question, as it's been asked before in various forms. A "P" in medicine is probably no big deal, depending on your grading system. Speaking from my own experience, it will be a problem for the "top" programs, but you can still improve with honors in your sub-I and strong letters from medicine faculty.

In my case, I didn't get honors in medicine or the sub-I, only had a couple honors in other rotations, zero in 1-2nd year, board scores were just above average, and I attended a middle tier state school. I didn't get interviews at my "top" programs (BWH, UW, U of C) but still matched at a program that's possibly top 10-15. You asked about getting into a "decent program"--if the rest of your application is in good standing, I'm sure you will have many choices.

I don't think getting a P on your medicine rotation will keep you out of the running for top 5-30 IM programs (I don't know what you would define as decent). For the top 5, you may need to get a strong step I score >245 to be on the safe side (if you go to a less prestigious school you have to shoot for a higher score), strong LOR, an honors for your sub-I, maybe an away rotation at the school where you want to match, AOA, +/- research. The top 5 programs are pretty selective and little dings here and there on your application add up. You can look at the interview stats to see what the average scores, grades and get an idea of where you can match or get interviews at. There are always outliers in the interview stats, but you can get a feel for the average.

Yes, because while an F in medicine is highly desirable, a P in medicine is still acceptable to most decent programs.
