Few questions,
1. Can we smile in the picture?
2. What background should I be using?
3. What kind of clothing? As in casual? Or should I wear a dress shirt of some sort??

These questions might seem silly but I just wanted to make sure.

It's just a 2" by 2" headshot photo. Wear something that doesn't blend with the background and you'll be fine.

1) At least look like you're happy to apply to their school.
2) Something simple and not too distracting to the viewer.
3) Something you would wear to an interview. Dress to impress.

My passport pictures make me look like I've just been dragged out of bed. That spaced out stare... the gaping open mouth.... the overall impression of who am I? What am I doing here? Who are you? 😴

If I get rejected it better not be because of my photo.
