how is this E? On E, the right side of there is a couple of edges, but on the figure, there is no edge (if you look at it with respect to the small edge at the bottom of the figure)

how do you differentiate between square and circles? can't see a difference.
 
The answer is A. If you look on the second fold, the bottom right corner remains where it is, it IS NOT folded inwards towards the center of the paper, thus it does not receive a hole punch. I Think you mistook the second fold as BOTH halves of the paper being folded inward, when it was only the top portion doing so.
 
Took a look. I agree with the software's answers for the first two but you make a good point on the last one. I can't differentiate between squares and circles just be looking at top view and side view.
 
The first question is not even tricky. If the bottom layer was folded, the bottom right edge of the paper/border of the paper would be shown as light grey/dotted. It is not though.
