I heard this book is great for pathology class and clinical system class. So, I start looking for it on Amazon. There is a user who commented the 4th edition is exactly like the 3rd edition. Yet, I see that the 4th edition has significantly more pages than the 3rd edition.

Does the 4th edition of Pathophysiology For Boards And Wards have significant updates/improvement over the 3rd edition?

You're not going to find many people here recommend either for path. Its essentially the pathology section of First Aid USMLE Step 1 scrunched into a pocketbook (i.e. its not nearly enough). Though admittedly, I did enjoy flipping through the 3rd edition book every now and then... pretty pictures. Its easy to carry around and skim through while you're eating or on the go.

But most of my heavy duty path was via RR and BRS (and, of course, pathguy.com and webpath).

I need a path book that is beyond just listing diseases and describing them because memorizing a list is not going help me solve 2nd order and even 3rd order questions. Therefore, I am searching for a book which compares the key findings between similar diseases in an organized fashion. Do you have any good suggestions?

Sorry, but wtf are you talking about? And you think pathophys from boards/wards is going to do that? No offense, but if you're relying on a source like boards & wards to grasp 2nd/3rd order concepts then you're not putting yourself in position to do very well on boards.

The best book that appears to answer your needs is First Aid. But again, FA is not enough if you intend to do well. I still stand by my rec of RR or BRS.
