A while ago I found a link here for a website that had all kinds of write-up/follow-up forms and stuff that you could simply print out and use in the wards. I can't find the website anymore. Does anyone have any idea of what I'm referring to?

I was going to post a link to mine but that Medfools one is about the same. Mine has more space for PE, Meds on the front.
