Seeing as how their application process page (http://www.salus.edu/od_traditional/od_application_process.html) doesn't make mention of a supplemental form, either, I think it's safe to say there is no supplemental. Lastly, I know the OptomCAS page ( http://www.optomcas.org/index.php?option=com_content&view=article&catid=5&Itemid=29&id=42) alludes to a supplemental, but the fact that there is no fee probably means, with all the other information in tow, that OptomCAS made a mistake.
