PDF of Autopsies for Board Application

So, is anyone else out there working on your board application? So for the part where it asks you: "List on separate page(s) all of the necropsies that you have performed, giving only age, sex, primary diagnosis, and date performed. Do not send complete autopsy reports. The minimum number of autopsies expected is 50."

My program uses the ACGME database which does make pdf reports but as far as I can tell, doesn't make one with only age, sex, dx, and date performed. Am I wrong, is it possible to make this report with the ACGME database? If this is not possible, are you all typing this up separately and converting to pdf or how are you going about this?

Doesn't have to be in pdf format. Just copy the age, sex, primary diagnosis, and date performed and paste it into MSWord for each one. The ACGME log is handy, if you have it updated, for reminding you which cases you did. If it's like my program was it is pretty easy to go into the database and pull up the case data that you don't have in the log.
 
Actually, now with the online application it specifies that you must upload the file in pdf format.
 
i listed all of my cases with only the required info in an excel spread sheet, then converted to a pdf. The ACGME case log system will (apparently) some day integrate this info into your application, but not this time around.
