Peds Sub-I year

After finishing my M3 peds rotation and peds electives, I'm 100% sure I'm going into peds. As far as M4/Sub-I year goes, I'd like to focus on useful months that will help out the most as an intern.

We have to do a month of Ambulatory (Planning on PER), Medicine (adult medicine, July to get it over with), Peds (think I'll use peds externship/sub-i wards month for that), Surgery, and Senior Seminar, then four electives. I'm very interesting in neonatology, so I want a NICU month. Of the following peds subspecialty months, what do the current interns/upper levels/practicing physicians think would be most useful in helping me transition to internship?


Ambulatory Clinics (worked during Gen Peds)
GI
PICU
HemeOnc (spent a week during gen peds, loved procedures)
Allergy/Immuno
Cardiology
Child Development Clinic
Endo
Neuro
Nephro
Rheum
Palliative Care (did a 2 week rotation this year, loved it)
Hospitalist

I'm not a resident, but part of the goal of 4th year is round out your education. I think any peds elective will be helpful for intern year, but don't forget that things like radiology or anesthesia might be useful/enjoyable as well.

I agree with the "round out your education" statement. While it is helpful to do several peds electives, you don't want to do your internship for free. This may be the last time that you will have an opportunity to work in other areas (radiology, derm, anesthesia, etc.), all of which might be useful for a future Pediatrician. So I would recommend taking a few Peds electives but taking a broader view for the rest. Which electives you take depends in part on the specialty areas that are particularly strong at your school or that provide good experiences for med students. There isn't an "agreed-upon" menu of 4th year experiences for future Peds interns. You should speak with your faculty advisor or your local residency program director for his/her suggestions.

As an MS4 I agree with Doctorveradoll and twilightdoc. Do things you may never have the chance to do again.....international elective, some other specialty like ENT, optho, path, radiology, teaching/prosecting. As a resident you will most likely have access to do electives in all of the pediatric specialities. Of the residents I've talked with most said they were glad they had done a NICU rotation or wished that they had.
 
I would also recommend trying to schedule rotations that you can take time off from in November and December especially. These are the biggest interview months. I would not want to be on a rotation that I really am enjoying or at a place where I am trying to showcase and have to be missing 2-3 days a week for interviews.

I would recommend also talking to those in the classes ahead of you. Some peds rotations sound awesome, but sometimes the MS4 doesnt get to do a ton unless its a sub-I. So definitely do your research.

Lastly, I would agree with trying to round out your education. I did adult ICU and pulm rotations, Anesthesia and ENT and EM at a rural, community hospital where its me and the attending. Radiology is helpful for everyone, but can be boring unless you have a good teacher. Derm is cool and there are a ton of rashes in kids. Path is always helpful, but try to make sure you are at a kid's hospital so you are not just seeing adult path all the time. Hope this helps.

this came out in november...its a short read about maximizing opportunities during 4th year to increase overall competence ( as opposed to lengthening residency).

Reference number 15 is a sample 4th year curriculum based on what you think your goals are. It has learning objectives, modules, and assessment tools. May be worth checkin out!
