Hey, everyone. Current 4th year trying to get the ball rolling on applying for licensure. The thing I'm not clear on is when I go to apply it gives me several options to choose for the application (pictured in attachment). Do I need to apply for each one separately, or is there only one I need to select? Any help would be greatly appreciated. Thank you

Why don't you just call the Optometric licensing board and ask them?
