Don't explain your transcript in your personal statement, at least not for PTCAS, for two reasons:
1) PTCAS has a limited number of characters for the personal statement, and explaining all that plus the rest of your statement won't give you much room
2) THere's a spot on the PTCAS application specifically asking about your transcript reflecting your abilities, and it gives you a place to explain it if necessary
Be creative somewhat, but not over the top...you want to stand out, but in a good way. I'm sure there are 1000 applications that are identical and along the lines of "I wanted to be a PT since I tore my [insert ligament/tendon here] playing [insert sport here] in [insert year] grade." Just write honestly...why do you want to be a PT? What appeals to you? How do you know it's the career for you?
I focused on the last question I just asked you...the "how." I changed my major 4-5 times in college, and I wrote about that, saying I thought I knew what I wanted, dove in and did internships, only to learn that it wasn't for me. But I got a job working as a PT aide and fell in love with it - I looked forward to getting up at 6am to go to work before a long day of classes, and didn't mind forgoing parties and bar nights because I had to work in the morning. I had never felt that way about any job before, so it made me step back and look at what I was doing.
I also focused a little bit on quirks, and why PT was always in the back of my mind (sure I had injuries, but there were other aspects of it that makes PT perfect for me).
I also had an admissions committee member from a school I wasn't applying to read my statement and edit it for me, which was super helpful. If you can find someone like that (I found this person on Twitter through all the PT accounts on there), or someone who has been accepted into PT school (better yet the program you want to go to), have them look at it and see what they think.
