For Step II, how do you ddx between
Histrionic vs Narcissistic PD: they both like to be center of attention and thinks they are important.
Histrionic is sexually seductive and uses physical appearance to attract attention.
Narcissistic thinks they are it and wants others to admire them and praise them.

Is there anything specific in a question stem that will give them away?

Secrets says:

Histrionic: overly dramatic, attention-seeking, and inappropriately seductive

Narcissistic: egocentric, lack empathy, use others for their own gain, sense of entitlement

Hope that helps 🙂

in USMLE:

narcissitic is usually a male, who dresses extravagantly and demands the best for himself. Since he's the best, he expects the expert, the specialist, and is usually emphasized (true of a both the UWORLD question and the one on my Step 2, of which I was off the chart to the right on Psych)

Histrionic is usually a female, with emphasis on the sexy slinky dress. Less focus is paid to the need for the best. The focus of the question is on being overly dramatic, especially in someone who is going to see you for some ridiculous complaint in middle age. Again, true of both UWORLD and the Step 2 questions).

Narcissistic: Dress and demands the best, uncaring about others

Histrionic: Dress and uses sexuality, cares what others think
