I want to re-enter the match next year, but will be a PGY-1. How in the world will this work with my current program? Do I have to resign first? Or should I try to arrange an out of match spot?

Why? This is not a smart-ass question. New specialty? Matched prelim only? Different program in same specialty? The answers to your questions will depend somewhat on the "why".

Really poorly. I interviewed for fellowships as an intern and even though I had the full support of my program, I still barely had time to make it to 8 interviews in a 3 month period. Expect the same.

If you want to ensure that you have a bad LOR from your current PGY1 PD and increase the risk that you won't have a spot next year, resigning is a good way to go. But it may also be the only realistic way, depending on the answers to my questions above. An out of match spot would be easier from a logistical standpoint...but won't give you much flexibility.

Sorry, I wasn't trying to be vague. Basically due to my own poor planning (believe me, I take full responsibility) I was not prepared to interview for my dream specialty even though from a numbers standpoint I'm competitive. I convinced myself that I would be "content" in another field, but I am now really feeling like I made the wrong choice. The program where I went to med school holds a spot out of the match every year for people in this situation, so I want to try for that spot initially. But if i don't get it, I'm not sure what my plan B should be since I really DONT want to leave my new program high and dry since this is my own mistake but I also don't want to be asking myself "what if" for the rest of my career. Thanks for your honest feedback.

...I interviewed for fellowships as an intern...

Wait, what!? A) why are you applying for fellowships with only a few months of clinical experience under your belt. B) what program would interview an intern for a fellowship spot. Are most fellowships based on Step1, grades, LORs just like residency?

ABIM Research Pathway. Look it up.

And the interviews l got for fellowship were even better than the ones l had for residency (which were pretty damn good).

I went on twelve fellowship interviews in a two-month span last year, and my program was NOT happy about it. At all.

If the people at your program are nice people/feeling generous then you might be able to work something out to have your lightest rotations scheduled during the interview season. Just don't count on it as a given.
Since I was in a one year internship where they knew that I would be looking for a different spot during the year, they didn't give me much hassle about going on interviews. In a categorical program where they don't expect you to be leaving you probably will get more resistance unfortunately.
