I want to re-enter the match next year, but will be a PGY-1. How in the world will this work with my current program? Do I have to resign first? Or should I try to arrange an out of match spot?

If you want to ensure that you have a bad LOR from your current PGY1 PD and increase the risk that you won't have a spot next year, resigning is a good way to go. But it may also be the only realistic way, depending on the answers to my questions above. An out of match spot would be easier from a logistical standpoint...but won't give you much flexibility.Do I have to resign first? Or should I try to arrange an out of match spot?
Why? This is not a smart-ass question. New specialty? Matched prelim only? Different program in same specialty? The answers to your questions will depend somewhat on the "why".
Really poorly. I interviewed for fellowships as an intern and even though I had the full support of my program, I still barely had time to make it to 8 interviews in a 3 month period. Expect the same.

...I interviewed for fellowships as an intern...
Wait, what!? A) why are you applying for fellowships with only a few months of clinical experience under your belt. B) what program would interview an intern for a fellowship spot. Are most fellowships based on Step1, grades, LORs just like residency?
