Some programs say that you're 'basically guaranteed a preliminary IM spot if you match with them', but when you go to the NRMP, there is still the option of making a supplementary list of preliminary year programs, and that school's preliminary year is only one of the choices.
My question is this: Do you have to attend a school's preliminary IM program if the NRMP offers the supplementary list option? I'm pretty sure I'd do the preliminary year at the Neurology program I match to, but I'd like to know if it's an option to do the preliminary IM year elsewhere, as a few preliminary IM programs have contacted me and told me I'm basically in if I want it..
Any help/advice would be appreciated!

Congrats on the prelim guarantees. Where they in major cities, solid programs?

Thanks, but that doesn't really answer the question..and we both know what a 'guarantee' before the match means...
Basically I'm wondering for programs that are supposedly 'advanced', but still tell you you'll match with their prelim IM program (although it's not formally attached), do you have the option of ranking other preliminary programs first? According to the NRMP you do, but I want to make sure you can, and that it doesn't somehow affect your chances of getting into that neurology program..

Are you saying that you don't want to take an advanced program up on it's offer to "guarantee" a PGY-1 slot, and wondering if that will impact your matching chances at that advanced program?

It isn't entirely clear from your post.

If a program is advanced, and not "attached" to a PGY-1 slot, then you can rank anyone you want for your PGY-1 slot. If they aren't attached then the PGY-1 match should not impact your advanced match at all. If you want to take them up on the offer of a free spot (which may or may not be true), then you'd want to rank their PGY-1 first. You would also want to let them know that you are counting on them to help you with this so-called "guaranteed spot", so they can tell the IM people that they want to use one of their dedicated slots for you. And you would want to tell them this without placing anyone in the position of violating any match rules.

This is a great question. I had the very same question regarding a couple of programs (Northwestern and Rush) that do the "You can do prelim year where you want, but we guarantee you a spot."

My ROL was completed before February because I'm couples matching and my significant other is out of the country the month of February, so I filled in Northwestern and Rush's IM prelim spot on my ROL, but with some trepidation.

Turns out my fears were justified. The correct codes for those programs were actually NOT the general internal medicine prelim codes. They had prelim/neuro codes set up that you can only link with their advanced program (Joint A/P). I don't think you can search for those codes. They emailed them to me a couple weeks ago. If you have any questions, I'd recommend emailing the residency coordinator for whatever program you have questions about.

Hey there, thanks for the information as always, I appreciate it! Not really sure yet if I want to take certain Neurology programs' preliminary IM spot or if I want to go to a prelim IM program I interviewed at (that was not attached to any advanced neurology programs). More than anything I just wanted to see what my options are.
One more quick question - do we have to create a separate rank order list for preliminary spots, or just a rank order list for neurology programs, with the preliminary IM programs attached? (if that makes sense.. 😳/)

Just a main match list and supplemental list(s) (they are labeled with Letters, starting with A). You can you supplemental list A for any and all advanced programs.
