I have tried to read through some of the threads. The way I am understanding it you can apply for cards fellowships in IM PGY2 and then again in IM PGY3. Is that correct?

Wow! What a sense of peace to land the fellowship in PGY2 so you know your future following PGY3. 👍

That is correct. You can also apply after PGY3 (i.e. after you've finished IM residency) but this isn't done as often.
