As a medical student, would you address a PGY4 as "Dr. Last name"?

Yes at first, unless they ask you to use their first name

In life it’s typically best to err on the conservative side and be politely corrected than to err on the liberal side and be rudely so.

I don’t mean anything political—I’m just referring to naming conventions here.
