I was just wondering if anyone on here has experience in doing 'house calls' as a pharmacist or knows of any ambulatory care pharmacists who do visit patients in homes. If so, what interventions are performed other than pill pox, adherence, device technique, etc and for what disease states?

Any help would be appreciated.

I expect this role belongs exclusively to nurses.
 
I have done some home visits in the past and I just did not the find the ROI to be worth my time. Too many other responsibilities and patients to see in clinic throughout the day to be able to go make home calls. We have social workers and nurses at the clinic who will report back to the me or call me from the home.
