PhD after M1?

Does anyone have experience with this? The program I'm (probably) matriculating to offers the option of doing the PhD after the 1st year, and it seems like it would be nice for continuity between M2/M3 ... does anyone have any advice one way or the other? The only downside I can really think of would be that your step 1 score might suffer?

Yep. The inevitable drop in your step 1 would not be offset by the marginal increase in knowledge carrying over from MS2 to MS3. Plus less time for rotations to find the right fit in lab.
 
The majority of MD/PhD programs do the PhD between M2 and M3. It's for a reason. M2 knowledge/memorization regurg ability is not so critical to be a successful M3.
