Why? Well, I'm pretty sure Dr. Kaye did his PhD during his residency at Tulane and moved up the academic ladder very quickly at various institutions afterward. I guess that's a reason why, climbing in rank; or really really loving research. I can't imagine doing it myself though.
I doubt you'll find many programs offering it since there is virtually no demand, but many places would probably accomodate your needs if you go for a PhD.

Currently I want to go into Academia. I think teaching would be fun! I had great Academic Anesthesiology mentors who made the field shines. I want to be able to show a lost medical student that one day in the future.
The earnings in PP are greater but material wealth does not equal happiness according to my personal experiences and my buddhist background. Karma. Dhamma... When money dictate one's action and patient care, one loses touch with medicine.
Loves basic science research and medical illustrations. The research time would definitely enhance that aspect of my interests...a more meaningful life for me personally in the long run.
There are also 2 year fellowships with a year of fellowship plus a year of research. It really depends on whether you want to work in a lab one day or teach in the OR.
