please help

I am currently a FM PGY1 and totally unhappy, last year I was debating bewteen FM and path and for location and personal reasons I went with FM. Now I realized FM is not really for me, and am kicking myself everyday. how difficult is to transfer to Path after a year of FM? do I need to participate in the upcoming match again? could I transfer outside the match, is that possible? thank you for your comments/help
 
We've had very many threads about this lately (not necessarily FM, but the other specialty doesn't really matter). There should be plenty of info in recent posts. In short, some people go through the match again, others get spots outside the match. Depends on who you talk to, where you are trying to go, etc. Start talking to departments you are interested in, and start talking to your own program director to work on letting you go.
 
I would guess that FM is vastly less competitive than Path training slots. I would think you might have a tough time matching. I personally have never heard of a FM trainee sliding over to path, IM from good programs/Surgery/OB yes on rare occasion.

I would attack the cause of your unhappiness rather than look for another job. Life is tough for IMGs now regardless of your field...

Make sure you are genuinely interested in Path. I don't know how difficult it is to transfer, but nothing is impossible. However, you need to talk with your program director and express your concerns. I don't know how your PD may respond and I don't know the circumstances, but you have got to talk to your PD and call up Path programs explaining your situation and see if something can be worked out.

In addition to talking with your PD, you have to show programs why you are interested in Path. The last thing they want is an unhappy resident from another program in their program just because they didn't like FM residency X. Did you do a path rotation? Do you have any path-related experience?

There are plenty of residents all over from different areas who switched into path - it has less to do with "competitiveness" of the field than it does with simply that many FM people don't switch into path. More likely to be hospital-based people like Surgery or IM, in my experience.

That being said, the above advice is also appropriate for your situation. Don't expect doors to open for you, you will have to be the proverbial squeaky wheel.

There is the whole Fresno vs. Bakersfield debate. Those are usually pretty hot and heavy.

Shiner Bock, Pat Green, BEEF, Mexican food.....there are a lot of great things about Texas. I certainly wasn't trying to rag on DF-Dub in the other thread. It's a great place to visit, but too big and too hot for me. That's all.

Another positive about DFW: it's not Houston! (just kidding).
 
Personally, I just have serious questions about a place (like Texas) that people always feel so obligated to defend and go into attack mode when people say anything that can remotely be considered as an "insult." Criticize almost any place in the country and people will say, "Well, you're entitled to your opinion, but I disagree, I enjoy living there." But say it about Texas and it's time to start shooting and any contrary opinions are (pick one) biased, uninformed, or jealous. It's like everyone in Texas is getting paid by the state to convince other people that it's a great place. People go out of there way to talk up Texas even when it isn't even warranted. I was in Monte Carlo once and I heard some ******* with a bolo tie start yammering to no one in particular about how Monte Carlo wasn't anything like Galveston Bay.

I'm from Massachusetts. I liked growing up there, it was a varied and interesting place. But when someone (like someone from Texas, who has never been there 😉 ) says it's hell on earth, that's fine, doesn't make any difference to me.

In light of this comment, just wanted to let you know, I switched from FM to Pathology this year, and even going outside the match, was offered 2 spots.

I couldn't be happier, now, but just as the others on here said, make sure of why you want to go into path... it's a demanding field. And you'll have to be assertive about looking for open doors, as well as confident in your new choice... but it can be done. You're welcome to PM me if you have questions!
 
I also know two people who switched from FP to Path - both wound up really hating path - one went back to FP - he's been practicing for years now and is a very happy family practitioner. Let's face it residency is sometimes a drag and it is always tempting to think the grass is greener. You really need to know yourself and what you really like.

It's definitely possible to change after one year, the question is how likely. Again that depends on your creds i.e. 1.AMG vs. IMG 2.Board scores. Unless you have a contact and get a prematch, you'd have to enter the match again. I started in IM, completed one year, and then switched into Path. It was a pretty miserable year though. There were times during conference I would rather have put a bullet in my head than listen to another lecture on smoking cessation. I feel your pain as I had to go through the same and hope things work out.

To the OP,

I matched into FM this year and left 3 days ago. I originally wanted pathology since my second year, but I am older and did not want to go through 4+1 year of fellowship. I showed up for the month of July, went to one clinic day and finished PALS and NRP. Rest was just orientation.....I thought I could muster the enthusiasm for FM somehow.....but in practice, I disliked the work. Now, I am doing Pathology rotation again at the same place I matched, lovin' every moment. Who cares if it takes a few years longer to work than FM. Bottom line...do what you like.

how did you tell your program and how did your program react when you left? will your 1 month FM affect your funding year?
 
My program director listened and understood what my reasons were. No bad feelings. I was in a large program, so they were able to adjust the schedule easily. As far as the funding issue goes...I am lost on this one. Not sure...anyone who does please reply.
