People can be (and in the case of S. pneumoniae, almost all people are) healthy carriers of the bacteria that conjugated vaccines are given more. Conjugated vaccines generate a robust response that includes mucosal immunity and can eliminate these normal bacteria that are just passively residing in the host. This clears out a niche for strains of S. pneumoniae not covered by the vaccine to take up residence in, and these bacteria can be problematic if they end up causing an infection, as we are not immunized against these strains.

Think of it similar to how clindamycin eliminates competitors and allows C. difficle to wreak havoc.

Couple other things related to this, that I am trying to recall:
-They will ask what is the difference between the pneumococcal vaccine given to a child and to the elderly; the child one has a more potent effect on T and B cells (World Q)
-The vaccine works only by protecting you from the capsule of the pneumococcal bug, if the strain does not have a capsule, it's going to be virulent in your body, they use the term "nontypable" strains (World Q)

Took my COMLEX 1 yesterday and lets just say those COMSAESs are a joke as compared to the real test. I scored 525 on COMSAE C but when I sat down to take my test yesterday, I felt no resource could have prepared me for yesterday's test. Too many neuro, OMM questions, questions that make you wonder how the heck did they come up with that vignette. I did COMQUEST but that was not as useful as I had expected it to be. The OMM questions were long and intertwined. You had to really speed up and read between the lines. There were no questions on biostats, renal, few endo, heme, MS. I am a little worried to be honest because I was scoring above average in COMQUEST . I felt so short of time that I had to finish the first 4 blocks without any break. My take on it is , get a good night sleep and really think through the options. That's the onlt thing you can do. Doing Savarese practice questions gives you a false sense of security because the OMM questions on the real test are way more lengthy and intertwined.

they arent virulent without their capsule though?

question: the nonconj vaccine is given to patients 2-65, and 65 and above and the conjugated is given to kids less than 2 year of age, Immunocompromised patients, and elderly > 65. is this correct?

Isnt the conjugate best for elderly and infants? That was what made most sense to me
