polymerization silicone???

Hello, I want to share with you an interesting observation. In the course of this year in our clinic we have three cases: after performing a vitrectomy on retinal detachment, was performed tamponade with silicone. Three patients after removal of silicone (tamponade lasting no longer than 2 months) have any of the secondary cataract (IOL ) silicone stuck to the posterior capsule and became very viscous. Attempts to remove when replacing the silicone failed. All three of these the patient was operated with the interval of 1 month. The silicone was from a different purchase batch, and of course, without any complaints about the packaging or the expiration date. At someone was such? What is it? The reason its polymerization?
 
What kind of IOL? What type of oil (1000, 5000cs, heavy oil)? Posterior capsulotomy present?

Silicone IOLs tend to have a lot of problems especially if posterior capsulotomy present in the setting of a silicone oil filled eye. Doubt this is emulsification given such little amount of time.
 
AcrySof IQ heavy oil, posterior capsule at the time of the first surgery was a whole. posterior capsulorhexis made during the second surgery because the lens diffusely clouded.
 
Strange, that's an acrylic IOL which usually does not have that issue. I have no experience with heavy oil as it is not available in the US. Not sure if that has something to do with it. There are various techniques I've seen to try to remove the oil including a hydraulic squeegee but sometimes requires IOL exchange. Interesting cases.
