I don't know for sure, but in theory any blood shunted to the caval system would have eventually made it to the right heart anyway (via portal system), so I wouldn't think it would significantly raise right heart preload. There may be a slight effect, since the blood is now going through a lower resistance circuit.
 
While its true there is a decrease in resistance I wouldnt expect it to have an impact on preload since this is merely replacing the high resistance portal htn.
