Position Swap --- What's the process?

Hello Forum,

I have found someone who would like to swap positions with me. We are both qualified residents. We are in different specialties. We are both PGY-1, and we are swapping for PGY-2.

Can anyone with experience with swapping tell us what we do next? How do we approach our program directors? What paperwork needs to be done?

Just for verification, all we need to complete the swap is both residents' and both program directors' approval. Is this correct?
