anyone ever switch into a combined program after doing a year of psych? do you have to start over even if theres a spot at a program for a second year? anyone know of such a spot?

It's possible to switch. Keep in mind that one feature of all combined med/psych programs is that you are basically an intern for 1.5-2 years. Typically you rotate back and forth between medicine and psychiatry rotations, and since you've already done a full year of psychiatry, if you switched over your PGY2 year would probably be a full year of being a medicine intern.
