Possible Academic IM? Do I have a shot?

Current OMS-IV
Career goals: hospitalist
Level 1: 608 (82nd percentile)
Step 1: 224
Taking Level 2 and Step 2 next week (240s on practice tests)
Comlex PE: first-time pass
Research: 2 publication and 1 poster last right before med school (lame I know). Doing a research rotation this Fall.
Class rank: 50%
All Ps on rotations (I really hate that my school is P for 10-80% on shelf exams).
Honors Rural Track
Four scholarships to attend national conferences throughout preclinical years
Average ECs and leadership
Great Sub-I in June with a community program with great feedback. Doing two more.
Letters from:
-Assistant PD from Sub-I offered to write one
-Hospitalist faculty member from a local program
-Private practice surgeon
-Departmental letter

My current problem is figuring out how many programs to apply to. Applying to WA, OR, ID, NV, AZ, NM, CO, MT, AR, KY, WV, NC, TN, VA
And I'll be honest, I've picked most programs based on location (rock climbing potential and family), but I'm having a tough time narrowing it down.
The current list is at 56 programs (my advisor told me to apply to 40 but I feel that's not enough)
I would be happy with community or university IM. Which university programs would I have a decent shot at within those states? I'm going to take UW and OHSU off the list.
 
I think you have a good chance at all the University programs in those states except for UW, OHSU, Duke, UNC, Vandy, CU and UVa. I'm assuming your list is a mix of community and university. If so, 56 is a very reasonable number and you should expect 15-25 interview invites.
 
He’s all pass at a DO school. You could be all pass with 250s at a big name MD school and have trouble.

That does worry me. It really does frustrate me that high pass doesn't exist here. I got 60-70 percentile on most nbme shelf exams and only get a passing grade for it.

Take a look at some of the university IM programs that offer rural tracks. You'd fit well into those.
