Anyone else verified have that weird indentation in their personal statement? All the sentences are indented except the first line of every paragraph.

Also I just realized the prompt for this year incorporates a brief message about why DO. How bad is it that I did not mention DO at all? I figured I'd go in more detail about why DO in secondaries.

Mine didn't have any indentation at all...I just have spaces between my paragraphs. It should be fine.

For not mentioning DO, I'm not sure. The prompt does ask about your influences and determination for wanting to be a DO, so it may be a problem if you didn't answer the prompt correctly. It is better to read the instructions next time before applying.
 
I just had spaces between paragraphs also, but after verification and viewing the application under manage my programs there was indentation.

Yeah, but even if I had read it there was barely any space to write about why DO. Next time I will read it more carefully, I just assumed if there was a legit prompt it would state that question in the application as well.
