young healthy patient s/p rad hyst. Gr 2 adeno, 3.5 cm tumor, outer 1/3 invasion, no lvsi, margins/paramterium/ nodes negative (12 removed). What do you offer?
 
For patients with adenoCA histology on GOG92 (Rotman update), 44% rate of recurrence without RT in the patient that met criteria for the trial. Granted, this doesn't quite meet the criteria, but 3.5cm is really close to 4cm. I'd offer pros and cons, and favor treating, and grade 3/4 adverse effects were really minimal. Nice little IMRT plan should do the trick.

Who here that interviewed at Downstate in the 90s and early 00s got pressed by Marv Rotman about where you were going to rank them? I felt like I was talking to a mob boss, getting an offer that I couldn't refuse. Man, the good old days. Now there are articles in the Red Journal about sticking to the rules. That is loose butthole.
