Has anyone had any luck get the practice DAT from the ADA? I have filled out the form multiple times online, and still have not received anything in the mail.

I filled out the online request form 3-4 times with no luck. I decided to call them and got it in the mail a few days later. The telephone number is (312) 440-2689
