There is an official STEP 1 practice exam (3 sections of 46 questions each) on USMLE website for 2012:

Recent test takers, can you comment on the similarity/difference/difficulty of this exam to the actual exam?

The questions are exactly the type seen on Step 1. In fact, the majority of the questions are classic test topics that appear every year. It's a high yield document and well worth doing. The NBME doesn't provide any explanations for the question set, but you can find explanations on this page: http://www.benwhite.com/medicine/explanations-for-the-official-usmle-step-1-practice-questions/
