Are spouses supposed to attend these things or just the interview candidates?

I've asked that question a couple of places and, yes, it includes your spouse.

I don't know if that's the case everywhere, though.

Yes. It does usually include your spouse.

If you have any question, you should ask the residency coordinator.

And IMHO, if your spouse is NOT invited to join the group, that ought to wave a big red flag for you regarding the program.
