Precedex for neuroprotection during cranis

Any of you dudes/dudettes has used precedex for neuroprotection during cranis?

Did a left frontotemporal cavernous hemangioma resection last week on a 30 y/o f healthy and my attending wanted to run precedex for neuroprotection.

I did not know that it had that application. I thought that's what the barbs were for. Only thing I remember was pt was bradycardic the whole time with the darn infusion.

Yes, there is some data on Alpha 2 agonists for neuroprotection but not very impressive so far:

That isn't an indication for precedex.

Avoiding hypotension and hypoxia is your biggest neuroprotective strategy for the crani.

You and your attending are ahead of your time.

Time will tell if dex is an adjunct to neuroprotection.
 
I would have asked the attending to show you ANY evidence available.

Dexmedetomidine PRESERVES brainwave/electrical activity in the brain...and PRESERVES sleep architecture better than the other sedatives we have....

that means that brain regulatory functions continue to function...how that would give protection better than a drug that turns off the brain would be a mystery to me.


Having said that....I've always said that just because it makes physiciologic sense doesn't mean that it will hold true, it's just that I don't believe any data exists.

If so...please post.

Not conclusive but worth consideration.


I think if I were about undergo a procedure where I may receive a neurologic insult, I think I would prefer to receive a more time tested neuroprotective agent....

But I've it said before....you can't go with what "makes sense"....we have to go with what the data shows (in humans)🙂
