I know this has been discussed but couldn't find the thread. Patient reportedly developed a rash from oral prednisone. Would you still do the epidural

Yes. Rashes from steroid do not appear to cross react with other steroids.

Thanks Steve..that's what I figured. Seems like I'm paranoid about everything now that I'm in the private world

Ok I just read what uptodate had to say on the topic. I theoretically could use a steroid from a different class i.e kenalog or dexa and be "safe" however they recommend formal testing prior to considering this as much is still unknown and cross reactivity is unpredictable. Hmmmmm I may play it super conservative on this one

Since they only got a rash from the other one, (which I dont totally believe anyway) you could just give them a modest oral dose of the other class of steroid and if they tolerate it but pain persists, proceed with ESI w same steroid.

I did it with dexa (which we all know doesn't work anyway 😉 ) and she did fine... I think I'm just paranoid no longer being under the governments umbrella

I usually just tell them it is cortisone not prednisone?
