I see that prelim IM are quite competitive since all the other advanced applicants are applying to them too (e.g. derm, ent). What can a neurology applicant do? In my readings on this forum, I see quite a few posts on people who managed to secure neuro spots, but not PGY-1. Any advice...?

Besides the obvious - applying to only categorical programs which is only 60% or so (?) of the total programs out there - not too sure. I guess apply to tons of prelim spots as well.

Figured... BTW, cat neuro programs are 54% of all programs, and cat neuro positions are only 50% of total positions.

Even though it might not be published as such, many neuro programs have a good working relationship with their respective IM departments, and if you really want to go to their program can usually help you by talking to the IM people. There are other programs in which the relationship is so good that the IM department will guarantee spots for the neuro people if they want them, usually in places where the neuro program is uber strong and they bring in people that the medicine department is happy to have around. Those programs won't be listed as "categorical", but it is a pretty much done deal.
