I am debating whether to rank prelim programs at my top advanced neurology programs vs. going to another city where I have a partner who cannot follow me to my neurology program's city for 2 years.

Obviously, the prelim program at the location of the neurology program will be very academic and get me well-oriented within the system as well as within the medicine dept.

The 2 prelim programs elsewhere that I am highly considering would be considered community hospitals by most measures but have academic affiliations with senior residents that cover that hospital over the prelims. Talking to their current prelims it does seem to have enough pathology managed in-house that would get me a strong medicine foundation. Their population seems to be diverse. Additionally, their turnover is on the higher side compared to other programs I interviewed at, leading me to believe that I would be prepared for time management and paperwork, etc.

Talking with a resident at a program that gives alot of prelim spots to pgy1 neuro residents, they told me that usually the residents that go elsewhere do tend to do okay once within the system, but he has seen multiple struggle.

I know that many neuro programs out there are still traditional advanced with no guaranteed spots for neuro prelims and people make it just fine every year, but if anyone has any advice, experience or opinion on this it would be really appreciated!

My advice is stay with your partner for a year. It seems like 2 years may not be a long time without your significant other, but it is. You should be able to adapt to change in environment if you're even halfway competent.
