I put NO for the question about ever having institutional/disciplinary action taken against you when it should have been a YES. I can't believe I missed that. Should I email all the schools I applied to about this ASAP or just the schools that are sending me secondaries?

Yes, it is an egregious mistake on your part that you must fix. Dishonesty with institutional or disciplinary action questions will not only bar you from this year's applications, but may also trickle down to successive cycles (eg. maybe a 3.8+/24 AA would have them look at you again, but still a low shot).

You have zero chance long-term by not properly informing every school you applied to. If you do nothing, and they find out (which is usually displayed on your academic records sent by the Bursars anyway, typically by the semester the infraction was received), you are done thinking about dental apps. There's always DO school.

What if it is something that is not on the transcript? I'm still disclosing it anyway, I can't believe it slipped my mind.

Academic and institutional infractions are always documented on a transcript or supplemental file, assuming you were charged with the violation. You have an extremely low chance getting away with this.

I ordered the official transcript and nothing was noted on there. I'm going to call the school to ask if there's anything in my disciplinary record.

Admissions committees run a background check and academic check on every person they plan to accept, before they send their offers. They may not immediately see it on your transcript in review, but you cannot escape your internal permanent file with a university.

Many dental schools require a Dean's certificate of good standing upon acceptance, so be aware of that.
 
You posted this twice? Well here is what I said in the other one:

If you did not get suspended or expelled and did not go to court and was charged with a DUI or drinking under the age or drinking in public, public introxication, etc. then you DO NOT need to report it. There is no reason to divulge this information as it will hurt your perception and chances for entrance. If they ever ask you if you've ever been drunk, passed out in public, or peed in a bush, then go ahead and be honest. Do not lie, but do not self-prosecute yourself when the question was never asked.

IA that shows on transcript: cheating, plagiarism, anything related to your academic agenda

IA that is provided by your Office of Student Conduct upon request by dental school: Any suspension, warning, or probation due to underage alcohol drinking, drugs, sexual assault, street fight etc.

Either way you must disclose it, unless your university has decided to expunge your record it will be up to you.

See if the conduct office still retains the record.
