Do you mean a position you just matched to and will not start until next year? If so, you're SOL. By entering the Match, you signed a contract stating that you would abide by its terms including going where you matched. There are a few ways around this but they're all basically suicide. You can ask NRMP for (but are not guaranteed) a Match waiver if you have a specific issue that precludes you from starting that position (matching to a program/city you don't like doesn't count). You can also just ask your incoming PD to let you out of your position but this will still count as a Match violation and you won't be able to Match in upcoming years.

Now, if this is a spot that you're finishing up year one of and you want out...just walk away.
