professor or GSI?

My letters of rec are written by the GSIs (graduate student instructor), kinda like a TA, and signed by the professor and the GSI I think since professors rarely write letters in my school. SO for the application I was wondering if I should put the prof.'s name and title (Dr.) and put the GSI's address? cause if I put the GSI's name the title would be Ms. or Mr., and if I don't even mention the prof's name once they see the letters they'r be like who the heck is this? so any opinions?
