Programs that Require additional information

Hey All,

I applied to a ton of programs and many require additional information. Rather than going through each program individually, I thought it may be better to crowdsource the information. By that I mean, add any additional information required by a program on SDN, so we don't all have to look it up. I'll start.

Arizona: Money and letter of interest
Minnesota: letter of interest
UTSA: eye exam
Kentucky: letter of interest

I'm assuming not everyone who applies to these programs gets asked for supplemental info?? I've had some programs ask for this info and not a word from others and my app was processed mid August. Anyone know if this is the case?
