Programs that require step 2 for ranking

Discussion on another thread has me confused about when I should take step 2.
Which programs require USMLE Step 2 to rank? Does this mean CS and CK or just CK?

VCU and Oklahoma both require step 2 CK to be ranked. i don't know about CS...anybody else?
